Country Care LTD Prescription Drug Order Process

To participate in this Farm Bureau member benefit, you must complete and mail in the Customer Profile form . This provides Country Care with contact, mailing and credit card information to establish your account. All prescription orders you place will be shipped to the address you provide, and billed to the credit card you provide. Orders can only be made by credit card.

Steps to Enroll
• Fill out the Customer Profile form and mail Country Care LTD, 5857 New Hope Road, Springdale, AR. 72762.
• If you cannot print the forms from your computer, just call 1-888-273-8631 to have them mailed to you.

Once you have completed the Customer Profile form, you can place orders for your prescriptions. You can place your order along with sending in your Customer Profile. Follow the steps below to "Order Prescriptions."

Steps to Order Prescriptions
• Check the availability and prices of the drugs available through Country Care LTD by clicking here.
• Check the country of origin of the drugs. Drugs shipped from the United States will have a $2.00 shipping fee. Drugs shipped from outside the United States will have a $15.00 shipping charge.
• Compare these prices, including shipping charges, to the prices you are currently paying. Print out this page for proof of pricing.
• If this comparison provides you savings, you are ready to place your order.

There are two ways to order prescriptions. By sending your prescriptions you are ordering your medications.

1. Mail or fax your prescriptions to Country Care LTD.

2. Have your Doctor fax your prescriptions to Country Care LTD.

You will be billed on your credit card for the cost of the drugs and shipping.

Steps to Refill Prescriptions
• Call Country Care LTD and place your order over the phone at 1-888-273-8631.

Receiving Prescriptions
If your medications are coming from the United States, you will likely receive them in 4-5 days. If your medications will come from Canada or Europe please allow 14 business days to receive your order. Please reorder your medications 3 weeks prior to the time you will need them.
